Immediate start, hybrid work model$83,968 plus superSupport key community partnerships and make a differenceThis is an amazing opportunity to join our Community Strengthening Team as a Community Support Facilitator to work across various portfolios such as homelessness, social housing, financial vulnerability, emergency relief and welfare support.Reporting to the Team Leader, Community Strengthening, you will play an integral role maintaining partnerships with community organisations to collaboratively deliver on Council’s social and health priorities including emerging issues.Your duties will include but not limited to the following. Co-ordinate Council’s response process for suspected cases of homelessness in conjunction with internal and external stakeholders.Provide information and advice, including referral pathways for residents experiencing emotional, financial, health, housing and other social welfare issues.Support the maintenance and liaison with residents in social housing owned by Council including new tenancyBuild and sustain relationships with relevant key health and social community organisationsSupport community groups to plan and implement initiatives that address identified health and social issues. About You If you are a passionate community advocate, someone who is accustomed to working in a complex service delivery environment and has experience in working with disadvantaged persons, individual groups and communities and looking to make a difference, this is the role for you! To be considered you will have the following:Qualification in Social Work, Community Services, Community Development, Counselling, Welfare or similar.Experience working successfully within, and liaising with other community agencies, welfare/housing providers and other relevant networks.Ability to respond to crisis and urgent matters that may arise when working with individuals and families who are homeless or at risk of homelessness or financially vulnerable.Ability to work collaboratively within a team environment with a desire to back up and support other agency programs as required including in times of emergency.Experience in sourcing and preparing grant or funding applications.About usCardinia Shire Council is a dynamic and progressive organisation.
